Below is the list of currently available forms. The first page of each form provides instructions for completing the form. Each form is stored in the Adobe Portable Document Format (PDF) and can be viewed and printed using Adobe Reader software. In most cases, the Adobe Reader software is already installed on your computer (you can tell by downloading a form and trying to open it; if it opens, you have the necessary software). If the Adobe Reader software is not already on your computer, you can download it from Adobe's website.
Instructions: Use this form to request payment / reimbursement for items and events associated with PAPA. Fill out the form in its entirety, attach all receipts and leave in PAPA box for authorization.
Instructions: Use this form to accompany any cash / checks being turned in for deposit. Please use one form for each event. List cash in total and each check separately.
Instructions: This form must be completed and submitted to the Business Office at least 2 weeks prior to the event date for any event to be held on school property.
